Our Health Our Care is the health and social care transformation programme for central Lancashire. We are working together with commissioners, Lancashire Care NHS Foundation Trust and Lancashire County Council to improve how health and social care services work in our local area.
Our Health Our Care is part of a wider transformation programme – Healthier Lancashire and South Cumbria, which is co-ordinating change right across our region. Healthier Lancashire and South Cumbria is one of 44 Sustainability and Transformation Plans (STPs) that are being developed to deliver the NHS Five Year Forward View and improve quality, develop new models of care, improve health and wellbeing, and improve service efficiency nationally.
Many of our staff are already actively involved in working groups that are designing new models of care – modernising how services work.
As you can see, we care greatly about the future of our local and regional healthcare and want to make changes now so that we are prepared for the future.
