In order for us to prescribe your medication while you are in hospital, we need to know what medicines you normally take. Please bring with you all medicines either prescribed by your doctor or bought from your pharmacy in their original containers. If you have a special card that gives details of current treatment please bring this with you also. This will help your doctor to prescribe medicines for you whilst you are in hospital. The pharmacist or pharmacy technician will also need to see your medicines.
Your medicines will be stored in a locked box beside your bed. The nurse has a key to this box and will administer most of your medicines from here whilst you are in hospital. Any newly prescribed medicines will be supplied from the pharmacy labelled ready for your discharge and they will also be locked in the box.
The pharmacist will check your discharge prescription when it is time for you to go home and a member of staff will explain your medicines to you before you are discharged. You will be supplied with at least 14 days supply of continuing medication or the complete course for antibiotics etc. Any medicines the doctor has discontinued will be removed prior to your discharge.
