What We Record

Information about you, your medical treatment and family background may be held on manual and computerised systems.  This information is vital to aid your treatment or the care of others.

Doctors, nurses and other members of the healthcare team need to make notes on paper or computer systems about any diagnosis, test results, treatments, drug prescriptions and other additional information that you may provide, that is considered to be relevant to the treatment of your condition or to aid the treatment of others. 

It is important that you do not withhold information as this may affect your treatment and wellbeing. 

Secretaries, receptionists and other clerical staff need access to some of your records in order to do administrative tasks, such as booking appointments and communicating with you. 

To manage your care when you leave hospital, some information we hold about you will be shared with other NHS professionals, such as your GP and community nurses, midwives and health visitors, as well as residential or nursing homes or social services.
